It is the most contested area covered on this site and the one where the evidence is clearest, in the sense that the central claim has been tested repeatedly and has not been supported.

This page describes the main forms, their origins, what happens in a session, and what testing has found. It also tries to explain why people report benefit, because that question deserves a real answer.

External qi therapy

This is the Chinese form and the one most relevant here. A practitioner is said to have accumulated qi through years of practice and to be able to emit it toward a patient, either through touch or with hands held at a distance.

The concept has old roots. Notions of a skilled practitioner affecting a patient's condition directly appear in traditional literature. But external qi therapy as a defined modality is a twentieth-century development, and it flourished particularly during the qigong boom of the 1980s.

That period is important because it produced the most serious attempts to test the claim. Chinese research institutions, in an environment that strongly favoured confirmation, conducted experiments on external qi emission and on related claimed abilities. Some reported positive findings. Under tighter conditions, the results did not replicate, and several prominent investigators publicly changed their assessment.

Reiki and therapeutic touch

Reiki was developed in Japan in the 1920s by Mikao Usui and reached the West through Hawaii in the 1930s. It is not Chinese and not Taoist, and it is frequently offered in settings that imply otherwise. Practitioners receive attunements from a teacher and then place hands on or near the recipient.

Therapeutic touch was developed in the United States in the 1970s within nursing, and involves a practitioner moving their hands over a patient to assess and modify a claimed energy field.

Therapeutic touch is notable for one of the more instructive experiments in this field. In 1998, a study designed by a nine-year-old for a school project tested whether practitioners could detect the presence of a human hand near their own without seeing it. Twenty-one practitioners were correct 44 percent of the time, slightly below chance. The study was published in a major medical journal and has not been convincingly answered.

What happens in a session

Sessions are quiet and typically last around an hour. The recipient lies clothed on a table. The practitioner works through a sequence of hand positions, either resting hands lightly on the body or holding them a short distance above it. There is little talking.

Recipients commonly report warmth from the practitioner's hands, tingling, heaviness, deep relaxation, and sometimes emotional release. Many fall asleep. These reports are consistent and there is no reason to doubt them.

The warmth is straightforwardly explained: hands held near skin do warm it, and this is measurable. The other sensations are consistent with what sustained attention to a body region produces, which is the same explanation given for qigong sensations elsewhere on this site.

What testing has found

The central claim is that something is transmitted from practitioner to recipient. This is testable, and it has been tested in several ways.

Detection studies ask whether practitioners can sense the claimed field. Results have been at chance level.

Blinded treatment studies compare real practitioners with people mimicking the procedure without training. Recipients generally cannot distinguish them, and outcomes are similar.

Physical measurement studies have looked for emissions of heat, electromagnetic radiation, or other physical quantities during treatment. Nothing beyond ordinary body heat has been consistently found.

Clinical trials of reiki and similar modalities show small improvements in pain and anxiety in some studies, of the size and pattern expected from placebo and attention effects, with better-controlled studies generally showing smaller or no effects.

Taken together, this is a reasonably clear picture. The specific mechanism claimed has not been demonstrated. The experience has real effects with ordinary explanations.

Why the reports are genuine anyway

Dismissing recipient reports as imagination is both unkind and inaccurate. Several ordinary things are happening and they are not small.

An hour of undivided attention from another person is rare and affecting. Touch, in a culture where non-sexual touch between adults is uncommon, has documented physiological effects. Lying still in a quiet room produces genuine rest. Expectation produces measurable changes in pain perception and autonomic activity. The ritual structure of the session — the specialness, the sequence, the practitioner's confidence — amplifies all of these.

People may genuinely feel better after a session. The important question is how the effect is explained, because that shapes what further claims can responsibly be made.

What practitioners are actually experiencing

It is worth taking seriously that practitioners are not, in general, dishonest. Most sincerely believe they perceive something, and the question of what they are perceiving deserves an answer rather than an accusation.

Several ordinary phenomena are available. Hands held still develop distinctive sensations — warmth, throbbing, pressure — from circulation and sustained attention alone, and these are exactly the sensations described. Small temperature differences across a body surface are genuinely detectable by the hand. Subtle changes in a recipient's breathing and muscle tone are perceptible to someone paying close attention, and an experienced practitioner may be reading these without knowing they are doing so.

There is also a well-documented general tendency: people who expect to perceive something in an ambiguous situation reliably do, and the perception is subjectively indistinguishable from ordinary perception. This is not a failing peculiar to energy practitioners. It affects everyone, including those assessing them.

The result is that a practitioner can have a vivid, consistent, and sincere experience of sensing a field while no field is present. That combination is uncomfortable but it is well established, and it explains the persistence of these practices better than either fraud or genuine transmission.

Why the explanation matters

If relaxation and attention are doing the work, then the practice is a good way of delivering rest and care, and should be described as such. If energy transmission is doing the work, then much larger claims follow: diagnosis at a distance, treatment of disease, effects on conditions that rest and attention could not touch.

That is why the mechanism question is not academic. Practitioners who believe they are transmitting healing energy will reasonably offer to treat serious conditions, and some do. The harm from that is not caused by the session itself but by what happens instead of medical care.

Distant healing

Some practitioners offer treatment at a distance, with no physical presence. This claim removes every ordinary explanation — no touch, no attention in the room, no rest in a quiet space — and leaves only the specific mechanism.

It has therefore been the cleanest thing to test. Studies of distant intercessory healing, including several large and well-designed trials, have not found effects. This is about as clear a negative result as this field produces.

The unfalsifiable defences

When negative results are raised, a standard set of responses appears, and recognising them is useful because each has the same structure.

That the effect cannot be measured by current instruments. This may be true, but it converts the claim into one that no evidence could ever bear on, which is a considerable cost.

One proposal is that observers who do not expect the effect may influence the experience. Because controlled testing requires observers who do not presuppose the outcome, this proposal is currently difficult to verify reliably.

That the practitioner was not sufficiently advanced. This can be applied after any failure and never before one, which is the signature of an unfalsifiable defence.

That the effect works on a subtle level not captured by the outcome measured. This is reasonable in principle but requires specifying what would be captured, and that specification is rarely offered.

None of these is dishonest, and people offering them usually believe them. But a claim defended by all four simultaneously has been placed beyond the reach of evidence, and at that point it is no longer making a factual assertion in any useful sense.

How to think about this sensibly

If you find a session restful and want to have one, that is a reasonable choice and nobody should be sniffy about it. Rest and attention are worth paying for, and the fact that the explanation is ordinary does not make the hour less valuable.

What is worth holding onto is the distinction between the experience and the account of it. Feeling better afterwards is evidence that something happened. It is not evidence about what.

The things to decline are diagnostic claims, treatment of diagnosed conditions, advice about medication, distant healing sold as treatment, and any framing in which reasonable questions are described as blocking the effect.

Key points

External qi therapy is a twentieth-century
