A herb is described by its temperature, its flavour, the direction in which it acts, the channels it enters, and its toxicity. A practitioner selects and combines herbs using these categories.

Understanding this system is necessary for reading anything about the tradition, and it is worth doing carefully, because the categories are neither arbitrary superstition nor a coded version of chemistry. They are a way of recording observed effects in a vocabulary developed before there was any other option.

The four natures

Every substance is assigned a thermal quality: hot, warm, neutral, cool, or cold. This is not the physical temperature of the material but a description of its effect on the person taking it.

The classification derives from observation. Dried ginger produces a sensation of warmth and increases peripheral circulation, and is classified as hot. Gypsum lowers fever and is classified as cold. Many warming herbs do in fact contain compounds that increase metabolic rate or dilate peripheral vessels, and many cooling herbs have anti-inflammatory or antipyretic activity.

The category therefore tracks something real, imperfectly. Where it goes wrong is when the label is treated as an explanation rather than a summary. Saying a herb reduces fever because it is cold is circular, since its coldness was inferred from the fact that it reduces fever.

The five flavours

Substances are assigned one or more of five flavours: sour, bitter, sweet, pungent, and salty. Each is associated with characteristic actions. Pungent disperses and moves. Sweet supplements and harmonises. Sour astringes and contracts. Bitter drains and dries. Salty softens hardness and moves downward.

Some of these assignments are literal, and the association holds up reasonably well. Bitter substances in the pharmacopoeia frequently contain alkaloids, and pungent ones frequently contain volatile oils. There is a real chemical pattern underneath the classification, though the tradition had no way to know it.

Other assignments are conventional rather than gustatory. A herb may be labelled with a flavour it does not obviously have, because its actions match that flavour's expected profile. This is a system that has partly detached from its original empirical basis, and it is worth knowing when reading it.

Direction of action

Herbs are described as ascending, descending, floating, or sinking. Ascending and floating substances act upward and outward, toward the surface and the head. Descending and sinking substances act downward and inward.

This category maps onto observable effects more directly than the others. A herb that induces sweating acts outward. A herb that causes purging acts downward. A herb that relieves cough by stopping the upward movement of qi is described as descending.

There is also a rough correlation with physical form recorded in the tradition: flowers and leaves tend to ascend, seeds and minerals tend to descend. This is a heuristic and the texts themselves note exceptions.

Channel entry

Each herb is said to enter one or more of the channels associated with organ systems. A herb entering the lung channel affects respiratory function; one entering the liver channel affects the domain the tradition assigns to the liver, which includes the eyes, the tendons, and the smooth movement of emotion.

This is the category that most requires care in interpretation. The organ names in Chinese medicine do not designate the anatomical organs of the same name. The kidney in this system encompasses reproductive function, bone, hearing, and the deepest reserves of constitutional strength. Translating it as kidney and reading it anatomically produces immediate confusion.

Channel entry is best read as a statement about which functional domain a herb affects, in the tradition's own map of function. It is a routing label, not an anatomical claim.

Toxicity and the grading system

The tradition records toxicity explicitly, and this is one of its more responsible features. Substances are marked as having no toxicity, slight toxicity, toxicity, or great toxicity, and these markings carry practical instructions about dose, preparation, and duration.

The system works reasonably for acute toxicity that appears soon after ingestion. It fails completely for slow cumulative damage, because nobody could connect kidney failure years later to a herb taken in the past. Aristolochic acid nephropathy is the clearest modern example: the herbs involved were not classified as particularly toxic, and the damage they cause appears after prolonged exposure.

This is not a failure of care. It is a structural limitation of observation without follow-up, and it applies to any pharmacopoeia built the same way.

How formulas are constructed

The tradition rarely uses single herbs. A formula is built with ingredients occupying defined roles, conventionally described using an imperial metaphor.

The sovereign herb addresses the main problem. The minister herbs support the sovereign or address secondary aspects. The assistant herbs moderate harshness, treat accompanying symptoms, or counteract side effects of the sovereign. The envoy herbs direct the formula toward a particular region or harmonise the whole.

This structure has genuine practical logic. Including a herb specifically to reduce the harshness of another is a real pharmacological strategy, and licorice appears in an enormous number of formulas partly for this reason. The claim that combinations produce effects unavailable from individual ingredients is testable, though it has been tested less often than it deserves.

Patterns rather than diseases

Herbs are not prescribed for diseases in the biomedical sense. They are prescribed for patterns, which are configurations of signs and symptoms. Two people with the same diagnosed illness may receive completely different formulas because their patterns differ, and one person may receive different formulas at different stages.

The strength of this approach is that it attends closely to the individual presentation, and patients frequently find the consultation more thorough than a brief clinical appointment. The weakness is that pattern identification has poor inter-practitioner reliability. Studies in which several practitioners assess the same patient have found substantial disagreement about the pattern and consequently about the treatment.

This is a serious finding and it does not get discussed as often as it should. A diagnostic system that different competent practitioners apply differently to the same patient has a problem that cannot be resolved by appealing to individualisation.

Preparation as part of the pharmacology

Processing methods are treated as altering a substance's properties, and in many cases this is demonstrably true. Herbs are stir-fried, roasted, treated with honey, wine, vinegar, or salt, steamed repeatedly, or charred.

Some of these processes have clear chemical effects. Heating aconite for extended periods hydrolyses its most toxic alkaloids into far less dangerous compounds, and the tradition's insistence on prolonged preparation of this herb is a genuine safety measure arrived at empirically. Vinegar processing alters the solubility of certain compounds.

Other processing claims are harder to defend and may be traditional convention. The category as a whole is one where traditional practice and modern analysis overlap unusually well.

Contraindicated combinations

The tradition records lists of substances that should not be combined, transmitted as rhyming mnemonics and known as the eighteen incompatibilities and the nineteen antagonisms. These are taught to every student and are observed in practice.

Their empirical basis is mixed. Some appear to record genuine adverse interactions observed over time. Others have been tested in modern laboratories without finding the predicted effect, and a few combinations on the forbidden lists are used in certain classical formulas anyway, which suggests the rules were contested even traditionally.

What the lists demonstrate clearly is that the tradition recognised interaction as a real phenomenon and attempted to systematise it. That instinct was correct. The specific content is a mixture of sound observation and inherited convention, and the tradition has no way to tell which is which.

What this framework can and cannot do

It can organise a vast body of material into something a practitioner can hold in mind and apply. It can record observed effects reliably enough that formulas remain useful across centuries. It can encode safety information about acute toxicity.

It cannot distinguish a substance that changes how someone feels from one that changes their disease. It cannot detect delayed harm. It cannot identify which ingredient in a fifteen-herb formula is doing the work, or whether any of them is. And it cannot correct itself, because there is no mechanism within the system for a traditional attribution to be shown false.

Those limitations are not reasons to dismiss the tradition. They are reasons to be clear about what kind of knowledge it contains.

Key points

Herbs are classified by temperature, flavour, direction, channel entry, and toxicity, and these record observed effects rather than chemistry.

The categories track something real but become circular when the label is treated as an explanation of the effect it was inferred from.

Organ names in channel entry do not designate anatomical organs and should be read as functional domains.

Toxicity grading works for acute effects and fails entirely for slow cumulative damage such as aristolochic acid nephropathy.

Formulas use defined roles, and including herbs to moderate the harshness of others is genuine pharmacological strategy.

Pattern identification shows poor agreement between practitioners assessing the same patient, which is a serious and under-discussed problem.

","

A
