There are hundreds of published studies and a substantial number of systematic reviews.

The honest summary is that the evidence supports modest benefits of the kind you would expect from gentle mind-body exercise, that these benefits are genuine and worth having, and that claims going beyond this are not supported. This page sets out what that means in more detail, including where the research is weak.

Why this is hard to study

Several methodological problems affect this entire literature and need stating before any results.

Blinding is essentially impossible. Participants know whether they are doing qigong, and so do the instructors. This leaves every trial open to expectation effects on both sides.

Control groups are genuinely difficult to design. A waiting-list control tells you only that doing something beats doing nothing. An active control such as walking or stretching is fairer but shares many of the same ingredients, so a null result may mean qigong works no better than walking rather than that it does not work. A sham qigong control is possible in principle but hard to construct convincingly.

Sample sizes are often small, follow-up is often short, and outcomes are frequently self-reported. Publication bias is well documented in this field, with a striking scarcity of negative trials from some research communities. Any reader should treat individual positive studies with caution and rely on reviews.

Balance and falls

This is the strongest area of evidence and the one with the clearest practical value. Regular practice improves balance measures in older adults, and there is reasonable evidence that it reduces falls.

This matters more than it may sound. Falls are a leading cause of serious injury and loss of independence in older people, and hip fracture carries substantial mortality. An intervention that reduces falls is doing something genuinely important.

The mechanism is unmysterious. The practice involves slow weight transfer, single-leg loading, controlled turning, and sustained attention to posture, which are precisely the components that balance training uses.

Blood pressure

Reviews consistently find reductions in blood pressure in people with hypertension, typically in the range of a few millimetres of mercury for systolic pressure. This is a real effect and comparable to other lifestyle interventions.

It is also modest. A reduction of this size is worth having as part of overall management and is not a substitute for medication where medication is indicated. Nobody should stop antihypertensive treatment on the strength of this evidence, and anyone considering changes should discuss them with their doctor.

Sleep, mood, and quality of life

Improvements in self-reported sleep quality appear consistently across studies. Reductions in anxiety and depressive symptoms also appear, with effect sizes generally in the small to moderate range.

These are the outcomes most affected by the blinding problem, since they are self-reported and highly susceptible to expectation. That does not make them worthless. It does mean the effects may be partly attributable to attention, routine, social contact, and the general benefit of doing something purposeful, rather than to anything specific about qigong.

Whether that distinction matters depends on your purpose. For a researcher it matters a great deal. For someone who sleeps better, rather less.

Chronic conditions and cancer supportive care

Qigong has been studied as supportive care in cancer, chronic fatigue, fibromyalgia, chronic obstructive pulmonary disease, and type 2 diabetes.

The pattern is consistent: improvements in fatigue, quality of life, and mood, without demonstrated effects on the underlying disease process. In cancer care specifically, there is reasonable evidence for reduced fatigue and improved quality of life during and after treatment. There is no evidence that qigong affects tumour growth, survival, or recurrence, and claims to the contrary are both unsupported and dangerous.

The distinction between supportive care and treatment is the single most important thing on this page. Helping someone feel better during chemotherapy is valuable. Suggesting it replaces chemotherapy is not a difference of degree.

Breathing, heart rate, and the autonomic nervous system

One component of qigong has a clearer mechanism than the rest, and it is worth isolating because it explains a good deal of what people report.

Slow breathing, particularly with an exhale longer than the inhale, produces measurable changes in heart rate variability and in the balance between the sympathetic and parasympathetic branches of the autonomic nervous system. Rates of around six breaths per minute produce the largest effect, and this is close to what qigong practice naturally settles into.

This is not a traditional Chinese finding and does not depend on any Chinese theory. It is ordinary respiratory physiology, and it has been replicated widely outside any traditional context. It gives a defensible mechanism for the reduction in physiological arousal that practitioners report, and it explains why the effect appears within a single session rather than requiring months.

It also implies something a little deflating: much of the immediate calming effect probably comes from the breathing alone, and a person who simply breathed slowly for fifteen minutes would get a good portion of it. The rest of the practice adds the postural, attentional, and physical-activity components, which is not nothing, but the breath is doing a substantial share of the work.

External qi

The claim that a practitioner can emit qi and affect another person at a distance has been tested repeatedly, including by Chinese institutions during the 1980s when there was strong institutional interest in confirming it.

Better-controlled studies have not produced replicable positive results. Where effects appear, they tend to disappear when the recipient does not know whether emission is occurring. This is exactly the pattern expected if the effect is one of expectation.

This is worth separating clearly from the rest of the evidence. Someone can reasonably conclude that qigong exercise is beneficial while also concluding that external qi emission has not been demonstrated. The two claims stand or fall independently.

Comparison with other exercise

A fair question is whether qigong outperforms ordinary gentle exercise. Head-to-head trials against walking, stretching, and conventional exercise generally show similar benefits, with qigong sometimes performing slightly better on psychological measures and similar on physical ones.

This is a reasonable place to land. Qigong appears to be an effective form of gentle exercise with an attentional component, comparable to yoga or taijiquan, and there is no strong evidence that it is uniquely effective.

Adherence is a genuine consideration though. The exercise people actually keep doing is the one that works, and for some people the structure, tradition, and low intensity of qigong make it sustainable when a gym membership is not.

Risks and adverse effects

Serious adverse effects are rare, which is a real advantage. Reported problems fall into a few categories.

Musculoskeletal strain, particularly knee pain from prolonged standing with poor alignment and shoulder problems from holding arms up too long. These are the commonest and are easily avoided with sensible progression.

Psychological disturbance, including agitation, insomnia, and in rare cases more serious episodes. These cluster in intensive unsupervised practice, prolonged retreat conditions, and people with existing psychiatric vulnerability. Anyone with a history of psychosis, mania, or dissociation should be cautious about intensive practice and should discuss it with a clinician.

The most serious risk is not physical at all. It is delay or substitution: someone with a treatable serious condition relying on qigong instead of seeking or continuing medical care. This has caused documented deaths, and no discussion of benefits is complete without saying so.

How to read claims you encounter

A few questions sort most claims quickly. Does the claim concern how someone feels, or does it concern disease outcomes? The first is plausible; the second requires much stronger evidence. Is the evidence a trial or a testimonial? Is the comparison against nothing, or against another activity? Is the source selling the training?

Be particularly alert to a specific rhetorical move: describing a small effect on a self-reported outcome, and then discussing it in language that implies an effect on disease. This happens frequently and is often not deliberate.

Key points

Blinding is impossible and control groups are hard to design, so the whole literature carries structural uncertainty.

The best-supported benefit is improved balance and reduced falls in older adults, which is practically significant.

Blood pressure reductions are real but modest and do not replace medication where it is indicated.

Benefits in chronic illness and cancer care are in fatigue, mood, and quality of life, with no demonstrated effect on disease outcomes.

External qi emission has not produced replicable results under proper controls, and this stands separately from the exercise evidence.

The most serious risk is substituting practice for effective medical treatment, which has caused documented harm.

","

Ink
